By accessing and using this website and our chiropractic services (collectively, the "Services"), you accept and agree to be bound by these Terms of Service and our Privacy Policy. If you do not agree to these terms, please do not use our Services.
These Terms constitute a legally binding agreement between you and our chiropractic practice. We reserve the right to modify these Terms at any time, and such modifications will be effective immediately upon posting to this website.
IMPORTANT: Use of this website and its content does not establish a doctor-patient relationship.
The information provided on this website is for general informational and educational purposes only. It is not intended as, and should not be considered, medical advice, diagnosis, or treatment. Always seek the advice of your physician or other qualified healthcare provider with any questions you may have regarding a medical condition.
A doctor-patient relationship is established only through an in-person consultation and examination at our clinic. Online forms, email communications, or website interactions do not constitute medical advice or create a treatment relationship.
Never disregard professional medical advice or delay seeking it because of something you have read on this website. If you think you may have a medical emergency, call 911 immediately.
Scheduling: Appointments must be scheduled in advance through our online booking system, by phone, or in person. Appointment availability is subject to change.
Cancellations: We require at least 24 hours notice for appointment cancellations or rescheduling. Late cancellations (less than 24 hours notice) or no-shows may be subject to a cancellation fee of up to the full service amount.
Late Arrivals: Please arrive 10 minutes before your scheduled appointment. Arrivals more than 15 minutes late may need to be rescheduled.
New Patients: First-time patients should arrive 15 minutes early to complete intake paperwork.
We are committed to protecting your health information in compliance with the Health Insurance Portability and Accountability Act (HIPAA) and California state privacy laws, including the California Consumer Privacy Act (CCPA).
Your protected health information (PHI) will only be used and disclosed as permitted by law and as described in our Notice of Privacy Practices, which you will receive at your first visit.
We implement appropriate physical, technical, and administrative safeguards to protect your PHI from unauthorized access, use, or disclosure.
Payment: Payment is due at the time of service unless other arrangements have been made in advance. We accept cash, credit cards, debit cards, and HSA/FSA cards.
Insurance: While we accept most major insurance plans, coverage varies by provider and plan. You are responsible for understanding your insurance benefits and any out-of-pocket costs.
Verification: We will verify your insurance coverage, but this does not guarantee payment. You are ultimately responsible for all charges not covered by your insurance.
Non-Covered Services: Some services may not be covered by insurance. We will inform you of any non-covered services before treatment when possible.
By scheduling an appointment, you acknowledge that:
- You will be asked to sign an informed consent form before receiving treatment
- Chiropractic treatment involves manipulation of the spine and other joints
- While rare, risks may include soreness, muscle strain, or in very rare cases, more serious complications
- You have disclosed all relevant health conditions and medications
- You may discontinue treatment at any time
